    If you are a NEW volunteer with the Orillia Minor Lacrosse Association or did not volunteer with us in the 2025 season, please use the linked form to request a letter from the organization for your VulnerableSector Check (VSC) application.

    OLA Offense Declaration Form
    All team personnel must have a Vulnerable Sector Check (VSC) on file. If you were a registered team staff member for the 2025 season AND you have registered again for the 2026 season, you do not need to complete another VSC. You may complete the attached Offense Declaration Form and submit it to our Registrar ([email protected]).
